Birthday party videography services are provided by an outside, authorized vendor.

Please only complete this form if you understand, can confirm, and/or agree:

• You have a confirmed booking with The Edge Rock Gym - Miami and have paid your deposit.
• Your guests will be informed in advance and agree to being recorded.
• Full payment will be due no later than 7 calendar days in advance of the event.

If our videographer is available, they will reach out with instructions to complete an agreement and make payment.
